PRP vs. Cortisone for Stubborn Plantar Fasciitis

Heel pain that won’t go away is frustrating. Plantar fasciitis—irritation of the thick tissue on the bottom of the foot—is one of the most common causes. Most people improve with stretching, better footwear, night splints, and activity changes. But when symptoms drag on for months, injections become the next step.

A 2025 review of 13 clinical studies (901 patients) compared two of the most common options: cortisone (steroid) injections and platelet-rich plasma (PRP). The results were clear: both treatments can help, but PRP had better pain relief in the medium term than steroid injections.

The review concluded that PRP is a safe and effective option and may be the preferred treatment for chronic plantar fasciitis that hasn’t responded to conservative care.
